CONSENT FORM
The Study on the Efficacy of Herbal Sex Enhancement Products offered by HerbalSex.com
You are invited to participate in a research study of herbal sex enhancement products. You were selected as a possible participant because you volunteered to do so and met our qualification requirements. We ask that you read this form and ask any questions you may have before agreeing to be in the study.
This study is being conducted and funded by HerbalSex.com, an on-line retailer of the subject nutritional supplements.
Study Purpose
The purpose of the study is to help HerbalSex.com confirm its internal research and development and choose the most efficacious products on the market.
Study Procedures
If you agree to participate in this study, we would ask you to do the following:
- Accept clearly marked samples of products delivered to you free of charge by HerbalSex.com;
- Test these products according to the recommendations provided with the shipment; and
- Report your observations in the on-line Survey, which will be provided by HerbalSex.com.
Risks of Study Participation
The products we offer are not experimental. Most of them are readily available on the market. HerbalSex.com is not aware of any foreseeable risks directly associated with consumption of the subject nutritional supplements. However, you should be aware that these products and statements contained in the product descriptions have not been evaluated by the U.S. Food and Drug Administration. These products are not intended to diagnose, treat and cure or prevent disease. Always consult with your professional health care provider before taking any medication or nutritional supplements.
Benefits of Study Participation
The benefits to study participation are:
- You can evaluate these products for own future use totally free of charge, and
- You get a 25% discount coupon for your next order.
Study Costs/Compensation
You will not incur any costs as a result of participation in the study and you will not receive any direct payment or compensation for you.
Confidentiality
The records of this study will be kept private. In any publications or presentations, we will not include any information that will make it possible to identify you as a subject. Your record for the study will only be reviewed by HerbalSex.com R&D Team with appropriate regulatory oversight. To these extents, confidentiality is not absolute.
Protected Health Information (PHI)
Your PHI created or received for the purposes of this study is protected under the federal regulation known as HIPAA. Refer to the attached HIPAA authorization for details concerning the use of this information.
Voluntary Nature of the Study
Participation in this study is voluntary. Your decision whether or not to participate in this study will not affect your current or future relations with HerbalSex.com.
